If you need us before or after our regular office hours due to an emergency situation, please call 911. If you need us for a non-urgent matter that does require after hours attention, please call our office at (863) 467-2159 and be connected to the on call physician.

To schedule an appointment, please call (863) 467-2159 during regular business hours.

Sick Visits - Please call to schedule an appointment before coming to the office. Our physician might be in the hospital, and we make every effort to see sick patients on the same day. If needed, we will schedule an appointment for a “walk-in” and will try to have a physician available.

Routine Appointments - These are for non-urgent, chronic medical conditions and medication refills. Please call in advance to get the appointment you prefer with your regular healthcare provider. We recommend you make your next follow-up appointment before you leave the office. Also, try to make arrangements to have any lab tests completed prior to your visit.

Referrals: Patients are responsible for obtaining referral authorizations from their primary care physicians. Please make sure you have your copy the day of your appointment. Patients without their referrals will need to be rescheduled.

Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.

New Patients

New patients are asked to arrive 30 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.


We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, Visa, MasterCard, American Express, and Discover.

You can also use our secure Online Bill Pay option to pay current statement balances online without setting up an account.

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

We recommend that you call your insurance company to verify that we participate with your plan. Please check with your insurance company regarding referrals, co-payments, and any deductible required at time of service.

Some insurances may require a referral for your visit to our office. Please make sure to contact your insurance company and/or your primary care physician to ensure that you do get a referral if it is required. Failure to obtain a referral, when necessary, may cause your insurance company to deny your claim.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We reserve the right to charge $25.00 for "no-show" appointments. We make every effort to stay on schedule, although emergencies arise.

Refill Requests

We only renew prescriptions that we initiated. For other medications, please contact the health professional who prescribed them.

We can fill many renewal requests by phone. Please contact our office at least 48 hours (2 days) before you expect to run out of medication. Sometimes, for medical reasons, we will need to see you before issuing the prescription.

Medical Records

A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, phone number, and fax number.

After-Hours & Emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (863) 467-2159 and our answering service will assist you.
